This policy explains how American Dental Health Providers (“ADHP”, “we”) handles information collected through this website, adhp.com. Protected health information created or received in the course of your care as a patient is governed separately by our HIPAA Notice of Privacy Practices, which is available at any of our offices.
Information we collect
- Information you give us. When you submit an appointment request or contact form, we collect the name, phone number, email address, preferred location, preferred contact method and any message you choose to provide.
- Technical information. Our web server and security provider record standard technical data such as IP address, browser type and the pages requested, used to keep the site available and to detect abuse.
How we use it
- To respond to your appointment request or question, and to contact you using the method you selected.
- To protect the website against spam and automated abuse, including rate limiting by IP address.
- To meet legal, regulatory and record-keeping obligations.
We do not sell your personal information, and we do not share it with third parties for their own marketing.

Email and web forms are not secure channels. Please do not include diagnoses, medications, images or other sensitive health information in a website form. Tell us you would like to discuss something and we will call you.
Service providers
We use a small number of providers to operate this site, including our web host and Cloudflare, which provides content delivery, security and the anti-spam challenge on our forms. These providers process data on our behalf. Videos embedded or linked from this site are hosted by Vimeo and YouTube, whose own privacy policies apply once you open them.
Retention
Appointment requests are retained for as long as necessary to respond to your enquiry and to meet our record-keeping obligations, then deleted. Anti-spam rate-limiting records are kept for one hour.
Your choices
You may ask us what information we hold about you, ask us to correct it, or ask us to delete it. California residents have additional rights — see Your Privacy Choices.
Children
This website is not directed at children under 13, and we do not knowingly collect their information through it. Where we treat a minor, information is provided by a parent or guardian in the course of care.
Changes
If we change this policy we will update the date above.
